My name is Rachel Clerge, and I am a board-certified Family Nurse Practitioner (FNP-BC) with 15 years of experience providing comprehensive medical care across a variety of healthcare settings. My clinical background includes primary care, urgent care, chronic disease management, preventive care, and wellness.
Throughout my career, I have cared for diverse patient populations in community health, long-term care, and inpatient hospital-based settings. In community health, I provided primary and urgent care to adults and adolescents, addressing a wide range of acute and chronic medical conditions. In long-term care, I cared for a geriatric population with complex medical needs and multiple chronic conditions. I have also provided medical care in an inpatient hospital-based psychiatric setting, performing medical evaluations, managing acute and chronic health conditions, and coordinating ongoing care.
These experiences have strengthened my commitment to compassionate, patient-centered care and to treating the whole person rather than focusing solely on a diagnosis.
At NeighborCare Practice, our goal is to provide personalized, accessible, and comprehensive healthcare through a holistic wellness approach. Our telehealth-based practice offers services including primary care, urgent care, hormone health, menopause and hormone-related wellness, and medical weight management.
My approach is centered on listening to each patient, understanding their individual health concerns and goals, providing education, and developing personalized treatment plans. I strive to create a welcoming environment where patients feel heard, respected, informed, and actively involved in their healthcare decisions.
